Here is the proper formatting for an address:
The Names always go on the first line. For the second line you'll need to decide if you want to abbreviate all the street types or spell them out- "Boulevard" vs. "Blvd." If you are abbreviating make sure to put a period after the street type: St. Ln. Blvd. Etc.....
If there is an apartment,unit or suite number you'll need to put a comma after the street type. Usually the apartment number will appear on the second line.
On the third line the city a state should appear. There is always a comma between the city and state. You'll need to decide if you're going to spell out the state or abbreviate all of them.
The zip code can go on the third line or it can go by itself on a fourth line. Some calligraphers will space out the zip code on the fourth line to fill out the space.

Note: If you're NOT going to abbreviate the state, you should NOT abbreviate the street type. Either go casual, or formal. Try not to mix.
